All
Parents are able to review materials, videos or call with questions prior to the
class.
If the parent does not want their child to participate in the classes, a note
must be written and sent to Mrs. White.
An alternative assignment will be given to the child in place of Family Life
Class.

Physical Exams:
Children should have subsequent medical examinations at least once during each developmental state, at early childhood (pre-school through grade three), pre-adolescence (grade four through six) and adolescence (grade seven through 12).
Annual Health Screenings will be performed on all grade levels. Height, weight, blood pressure, vision and hearing will be done. Scoliosis Screenings are done biannually on children age 10 and above. Parents will be notified if any concerns are noted.
Vision Screenings are not a substitute for an actual eye exam by an Eye Doctor.
Immunization Requirements:
Please speak to your Healthcare Provider regarding your child’s immunizations.
Kindergarten-----DTAP, MMR and Polio are required for entry into Kindergarten, after the age of 4.

New Immunization Requirements For 6th Grade Students:
Grade 6, 1 dose for students entering Grade 6 on or after 9-1-08 and born on or after 1-1-97. A child does not need a Tdap dose until five years after the last DTP/DtaP or Td dose.
For pupils entering Grade 6 on or after 9-1-08 and born on or after 1-1-97. 1 dose.
Always bring in documentation from your child’s physician’s office when any new immunizations are given. These dates will be added to your child’s New Jersey State Public School Health Card.

NJ Family Care is federal and state funded health insurance program to help uninsured children and low income families quality insurance. The website is www.njfamilycare.org. Application can be done online.
Gym Excuses:
All Gym Excuses for illness or injuries requires a parent note and physician’s note if limitations are advised. A written assignment will be given for inability to participate in gym.
Fever:
If your child is home with a fever, please keep home until they are FEVER FREE FOR 24 HOURS with out the use of Motrin or Tylenol.
